Payment for evaluation must be made in full prior to the start of the evaluation if:
1. driver is required to undergo the evaluation by request of a medical doctor/physician
2. driver is required to undergo the evaluation by request of the hospital (must be in current care).
3. request for evaluation is made by the family member (senior drivers).
4. driver is applying for vehicle modifications funding
